Bankruptcy Qualifying Form & Over-the-Phone Option

Unsure Whether You Should File Bankruptcy?

We can let you know whether you qualify either over the phone or in person. Before meeting with the attorney and in order to better advise you, we ask you to complete a two-page information sheet. This form is available below.

Bankruptcy Form

1. Click to download either form:
MS Word 2003-07: Bankruptcy Client Form. doc * (Works/WordPerfect/Open)
MS Word 2010+: Bankruptcy Client Form. docx *
2. Choose ‘Save file as’
3. Fill out form
4. E-mail it to us

The attorney will use it to advise you. To our bankruptcy clients, we offer the option to meet over the phone 909-888-7895 or at our office (San Bernardino or Glendora). The sooner we receive your completed form the sooner we can help. Find out if you qualify today. We hope this will ensure that you have a convenient and effective visit with us.

*Upon opening the document in Word, click Enable Editing. If it’s in a ‘read-only’ format, simply ‘Save As’ a new document; that will make it editable. Be sure to replace the ‘$value’, ‘#’, ‘$amount’, ‘description’, etc. with your information. Estimate where necessary. Call us should you have trouble.
